We will be changing the way we renew prescriptions for our patients from September 1st 2017. We are doing this in an effort to make it safer to renew your prescription and easier too.
We will no longer be able to take prescription requests over the phone and are asking our patients to fill out this form and either drop it in to us or e-mail us a picture of it. If you prefer, you can e-mail a typed list directly to us, to email@example.com.
For those patients who already are on a list of medications, when we renew your prescription we will attach a list to it that you can return to us when it is time to renew your prescription. All you need to do is tick the medications you require and alert us to any changes that may have been made by hospital based consultants.
All prescriptions will be ready after 24h and occasionally we may be unable to fulfil your request as we need to see you to arrange a blood test or check your blood pressure-we will let you know if this is the case.
